When Can You Fly After a DVT?

You've had a DVT, treatment has started, and now there's a trip coming up. It's a fair question, and one that comes up constantly: is it actually safe to fly?
Why You May Hear Different Answers
Different medical and aviation guidelines don't give exactly the same answer. The British Thoracic Society recommends delaying air travel for two weeks after a DVT or pulmonary embolism. The International Air Transport Association takes a more individualized approach for DVT, allowing travel once the patient is asymptomatic and stable on anticoagulation.
That difference is important. It means there isn't one universally accepted countdown that applies to every patient — but it also doesn't mean timing doesn't matter. How recent the clot is, whether you're stable on anticoagulation, and how you're doing clinically all factor into the decision.
Sitting Still Isn't Nothing, Even With Treatment
Anticoagulation helps prevent the existing clot from extending and reduces the risk of new clot formation. But prolonged sitting still slows blood flow in the legs — the same basic mechanism behind travel-related clots in anyone. Being appropriately anticoagulated substantially changes the risk calculation, but it doesn't make prolonged immobility irrelevant.
If you're already taking treatment-dose anticoagulation, don't add extra anticoagulant medication for the flight unless your treating clinician specifically tells you to.
What Actually Helps If You Do Fly
On longer flights, get up and walk periodically when it's safe to do so, and move your ankles and calf muscles while seated. Staying reasonably hydrated is sensible for general comfort, but hydration itself hasn't actually been proven to prevent travel-related blood clots — worth knowing, since it's often repeated as if it were.
When It's Worth Waiting, Even Without One Fixed Rule
If your DVT was diagnosed within the last two weeks, this deserves particular caution — the British Thoracic Society specifically recommends delaying air travel during that period. Other guidance allows a more individualized decision once you're asymptomatic and stable on anticoagulation. If you're still significantly symptomatic, clinically unstable, or not yet established on treatment, the decision should be discussed directly with the clinician managing your DVT rather than decided from a general rule found online.
Vascular Surgeon's Take
I don't look at the calendar alone. Timing matters — particularly early after a DVT — but so does whether anticoagulation is established and whether the patient is clinically stable. Two patients at the same number of days after diagnosis may still need different advice for the same flight.
The Bottom Line
There isn't one waiting period used by every guideline. The British Thoracic Society recommends delaying air travel for two weeks after DVT, while IATA guidance allows travel once a patient with a recent DVT is asymptomatic and stable on anticoagulation. In practice, the right timing depends on how recent the event is, whether treatment is established, your symptoms, and your overall clinical stability. If you're planning to fly soon after a DVT, get individualized clearance rather than relying on a generic number found online.
