In short: Varicose veins develop when tiny one-way valves inside your leg veins wear out, letting blood pool instead of flowing back up to the heart. Most cases are more of a nuisance than a real danger, but a specific pattern — pain, skin changes, bleeding, or anything suggesting a clot — is worth getting checked. Diagnosis is a simple, painless ultrasound, and treatment today looks nothing like the surgery your parents or grandparents might have had.

What's Actually Happening Inside the Vein

Think of your leg veins as a one-way street system, working against gravity to push blood back up to your heart. Tiny valves inside the veins act like doors that swing open to let blood through, then snap shut so it can't slide back down. When those valves wear out or stop closing properly, blood starts to pool in the vein below them instead of moving on. Over time, that pooled blood stretches the vein wall, and the vein starts to bulge and twist — that's the varicose vein you can actually see.

Doctors call the broader condition chronic venous insufficiency, or CVI for short — and varicose veins are just the visible part of it, not a separate problem. There's a scale doctors use to describe how far along this is, called CEAP: it goes from tiny visible thread veins, to full varicose veins, to swelling, to skin changes, all the way to open sores near the ankle in the most advanced cases. You don't need to memorize this — just know that if your doctor mentions a "stage" or "class," this is the scale they're using.

Cosmetic Concern, or Something More?

This is usually the first question people have, and the honest answer is: it depends on where things stand. Varicose veins are extremely common — roughly a third of adults have some form of them — and for most people, especially early on, they really are mostly a cosmetic and comfort issue, not a health threat.

That said, "usually harmless" doesn't mean "never worth attention." As the condition progresses, it can start causing real symptoms: aching, heaviness, throbbing, and leg cramps at night, especially after a long day on your feet — and usually a bit better once you lie down and elevate your legs. Left alone for years, more advanced cases can lead to skin discoloration and, eventually, slow-healing wounds near the ankle — a much bigger deal than the cosmetic issue that started it all.

Why It Happens

The same factors show up again and again: a family history (this runs in families more than people expect), being female (hormones play a real role here), pregnancy (extra blood volume plus hormonal changes — which is why varicose veins often show up or get worse during pregnancy), jobs that involve a lot of standing or sitting still, getting older, and carrying extra weight. Having several of these at once adds up fast, not just a little.

When It's More Than Appearance: Warning Signs

A few signs mean it's time to stop managing this on your own timeline and get it looked at. Sudden pain, swelling, or tenderness right in the vein. Skin near the ankle that's changed color, gotten thicker, or developed a dark, leathery look. Any bleeding from a varicose vein — it can happen from surprisingly small bumps, because these veins are thin-walled and under more pressure than normal. A vein that feels warm, hard, or like a tight cord under the skin, which can mean a small clot has formed right in that vein — something worth having checked, since this kind of clot has more overlap with deeper, more serious clots than doctors once assumed, especially when it sits close to the groin or behind the knee.

And this is worth saying clearly: sudden swelling in one leg, along with warmth and pain, can also be a sign of a blood clot in a deeper vein — a different, more urgent problem than a varicose vein itself. The two can look similar at first glance, which is exactly why an ultrasound, not guesswork, is what actually sorts it out.

How It's Actually Diagnosed

The go-to test is a duplex ultrasound — the same painless, wand-on-the-skin ultrasound used in plenty of other contexts, just aimed at your leg veins this time. It lets your doctor actually watch blood flow in real time and see exactly which valves aren't closing properly. Current medical guidelines are clear that this ultrasound should be a standard part of evaluating varicose veins, not something reserved for complicated cases. It's how your doctor figures out exactly which veins are the problem, which matters for deciding what treatment actually makes sense.

The Treatment Landscape: What's Actually Changed

Here's something worth clearing up, because a lot of people are working from an outdated picture. If you're imagining varicose vein treatment as a real surgery — general anesthesia, cutting the vein out, days of recovery — that's not how it typically works anymore. That older approach, sometimes called vein stripping, is largely a thing of the past.

Modern treatment is far simpler, and current medical guidelines are clear about which approach they recommend as the first choice: closing the faulty vein from the inside using heat, rather than removing it surgically. Here's what that actually looks like. A very thin tube is guided into the problem vein through a small needle puncture — no incision, just a pinch, similar to having blood drawn. You're awake the whole time, with only the area numbed. Once the tube is in place, it delivers a controlled dose of heat (from either a laser or radiofrequency energy — two slightly different tools that accomplish the same goal) along the length of the faulty vein. That heat causes the vein to seal shut.

This might sound concerning — "won't closing off a vein cause a problem?" — but it doesn't, because your leg has plenty of backup routes. Blood simply reroutes through the healthy veins nearby, the same way traffic reroutes when one street closes. The faulty vein, no longer carrying blood, gradually fades and is absorbed by the body over time.

For the smaller, bulging veins you can actually see under the skin — the ones branching off the main problem vein — there are two common options. One is removing them through tiny nicks in the skin, small enough to not need stitches. The other is an injection that closes the vein from the inside, similar in concept to the heat treatment but delivered as a liquid or foam instead. Which option makes sense depends on the size and pattern of the veins, something your doctor determines during the ultrasound evaluation.

What About Medication?

Not everyone needs — or is ready for — a procedure right away, and that's where medication can help. A category of oral medications called venoactive drugs (a common one is MPFF, a purified flavonoid compound) can meaningfully ease symptoms like heaviness, aching, and swelling, with real clinical trial evidence behind the effect. Current guidelines suggest these specifically for people who aren't candidates for a procedure yet, are waiting for one, or still have symptoms afterward — it's a genuine option, not just a placeholder, though it manages symptoms rather than fixing the underlying valve problem. Benefits typically build over one to two months, and longer courses (three to six months) are sometimes used for more significant symptoms.

What the Day of Treatment Actually Looks Like

Most people are in and out in under an hour, fully awake, with just the treatment area numbed — no general anesthesia, no hospital stay. And here's the part that surprises almost everyone: you're expected to walk right afterward. It's not just allowed, it's actively encouraged, because moving your legs helps blood flow smoothly through the reroute your body is now using.

Some patients are also given a short course of blood thinners afterward — often around 48 hours, sometimes longer. This isn't routine for everyone; it's a precaution used more often in patients with additional clotting risk factors, since heat-based closure can occasionally extend slightly into the deep venous system right where the treated vein connects to it, which is specifically checked for on a follow-up ultrasound.

Recovery, Realistically

Some bruising and soreness along the treated area is normal and usually settles within a week or two. You'll likely wear a compression stocking for a while afterward — how long and how snug depends on your specific situation, and it's worth asking directly rather than guessing. Most people are back to normal daily activities within a day or two, and back to full exercise within one to two weeks.

One honest thing worth knowing upfront: varicose veins can come back, even after successful treatment — usually new ones, from the same underlying valve weakness showing up somewhere else in the leg. That's not a failed treatment. It's just the nature of a condition that comes from an ongoing tendency in your body, not a one-time glitch that gets permanently fixed.

Vascular Surgeon's Take

The thing I find changes a patient's decision more than any statistic I could quote is realizing that treatment today looks nothing like what a parent or grandparent went through. Most people are picturing that older surgery — real anesthesia, real recovery — and that mental image alone keeps some people avoiding a simple evaluation far longer than they need to, for something that's typically an hour-long visit you walk out of on your own two feet.

What the Evidence Shows

Current care is based on joint guidelines from the Society for Vascular Surgery, American Venous Forum, and American Vein and Lymphatic Society, updated in 2022-2023, which recommend ultrasound evaluation for everyone with varicose veins and favor heat-based vein closure over traditional surgery for the main affected vein — a strong recommendation based on solid evidence. Among venoactive drugs, MPFF and Ruscus extracts carry the strongest evidence base, studied in multiple double-blind, placebo-controlled trials. The CEAP severity scale, most recently updated in 2020, is the internationally standardized way doctors describe and track this condition. The Brazilian Society of Angiology and Vascular Surgery has also published its own national guideline, reflecting the same evidence and approach described here.
