Sitting Too Long? Your Legs Could Be at Risk — What Really Happens During Prolonged Immobility

In short: Sitting still for long periods switches off the calf muscle "pump" that keeps blood moving in your legs. In most healthy people this alone isn't enough to cause a clot, but combined with another risk factor, it can lead to Deep Vein Thrombosis (DVT). Watch for one-sided leg swelling, warmth, redness, or pain; get up and move every hour; treat sudden shortness of breath or chest pain as an emergency.

Long flight? Long day at your desk? Your legs may be paying a price you can't see. Hours of sitting slow down blood flow in your legs — in some cases, enough to trigger DVT.

The Core Problem: Your "Second Heart" Shuts Off

Your calf muscles work as a second heart for your circulation. Every step squeezes the deep veins and pushes blood back toward your heart; one-way valves stop it sliding back down — the calf muscle pump. Sit for hours and this pump switches off. Blood pools in your lower legs, and pooled blood is more likely to clot.

Immobility alone rarely causes a clot in a healthy person. Clot formation typically requires a combination of factors known as Virchow's triad: stasis (slowed flow, what sitting causes), hypercoagulability (an already-elevated tendency to clot — dehydration, hormonal contraception, genetic disorders, cancer, recent surgery), and vessel wall injury (more relevant after surgery or trauma than simple sitting).

What the Numbers Actually Show

The WHO's WRIGHT project found that travel of 4+ hours roughly doubles relative VTE risk, rising further with each additional hour. Even so, absolute risk for a healthy traveler stays low — roughly 1 in 4,600 to 6,000 long-haul flights. Risk concentrates heavily in people who already carry another risk factor: in several studies, more than 80% of travelers who developed a clot had at least one — recent surgery, previous VTE, hormonal contraceptives, obesity, pregnancy, or an inherited clotting tendency.

Most dedicated research focuses on flights because that's where the best data exists — but the underlying mechanism applies equally to long car rides, hospital bed rest, and sedentary workdays.

Warning Signs

One-sided swelling, warmth, redness, or a deep ache that doesn't ease. Two or more together — see a doctor the same day. Sudden shortness of breath, sharp chest pain, a racing heart, or coughing up blood signal a possible pulmonary embolism — call emergency services immediately.

Keep the Pump Switched On

Stand and walk 1–2 minutes every hour. Flex your feet and rotate your ankles while seated. Stay hydrated; go easy on alcohol. Ask about compression stockings if you're at elevated risk — evidence supports their use specifically in higher-risk groups, not as a blanket recommendation.

What the Evidence Shows

Multiple studies (including a cohort of 8,755 international organization employees followed for nearly 39,000 person-years) confirm the WRIGHT findings using objective ultrasound data, not self-report. Cochrane review evidence supports compression stockings specifically for airline passengers with elevated baseline risk.

Vascular Surgeon's Take

The detail that changes my advice more than total sitting time is whether the immobility is continuous or broken up — even a 60-second stand-and-flex every 45 minutes measurably changes venous flow compared to the same total sitting time taken in one unbroken stretch. I'll ask patients specifically about their pattern, not just their hours, because two people with identical desk jobs can carry meaningfully different risk based on that alone.
