Scope note: This article looks at pregnancy strictly through the clotting-risk lens — the same way I'd assess any elevated-risk period in clinic. What follows is the vascular side: what's happening in your blood, and what's worth knowing regardless of that conversation.
Why Pregnancy Changes Your Blood
Pregnancy is one of the strongest clotting risk factors that exists. Three things happen at once: your blood becomes naturally more prone to clotting (a built-in protection against bleeding during delivery), your growing uterus puts pressure on the veins returning blood from your legs, and hormonal changes slow that return flow further. Think of blood like water in a hose slowly turning to syrup — it still flows, just more sluggishly, and thicker, slower-moving blood is more prone to clotting. Pregnancy adds more contributing factors to that mix at once than most situations we cover.
The risk doesn't end at delivery. It stays elevated through the postpartum period — the first several weeks after birth carry some of the highest relative risk of the entire pregnancy.
The Honest Numbers
Pregnant women have roughly a 4- to 5-fold higher risk of venous thromboembolism compared to non-pregnant women of the same age. That risk climbs further in the postpartum weeks. It sounds alarming, but absolute risk stays low for most healthy pregnancies — this is about relative risk, not a prediction that it will happen to you.
What Raises the Risk Further
A personal or family history of clots, a known clotting disorder, being over 35, a higher BMI, multiple pregnancy (twins or more), prolonged bed rest, and cesarean delivery all add to the baseline pregnancy risk. If you have several of these together, it's worth a specific conversation with your OB/GYN about whether additional precautions make sense for you.
Warning Signs Worth Knowing
The same core signs apply: swelling in one leg (a normal amount of pregnancy swelling is usually both legs — one-sided is the flag), warmth in one area, redness, or a dull ache that doesn't ease. Sudden shortness of breath or chest pain is different — that's an emergency, call for help immediately, and don't wait to see if it passes.
What's Actually Worth Doing
Stay as active as your pregnancy allows — short walks, ankle rotations during long periods of sitting. Stay hydrated. Mention any personal or family history of clots to your OB/GYN early, not after something happens. Know that some swelling is normal in pregnancy — the goal isn't to panic at every ankle that puffs up, it's to recognize the specific one-sided pattern that's worth a same-day call.
Vascular Surgeon's Take
The specific window I flag more than patients expect isn't the pregnancy itself — it's the six weeks after delivery, particularly the first one to two. I've had patients discharged after a completely normal delivery who assumed the "risk period" ended when the pregnancy did; it's a detail that needs to be said explicitly, because nothing about a normal postpartum recovery makes it obvious on its own.
The Bottom Line
Pregnancy genuinely raises clot risk, through mechanisms your body puts in place on purpose. For most healthy pregnancies, that elevated risk doesn't translate into a clot — but knowing the one-sided warning pattern, and flagging your own risk factors early, is the part that's actually in your control.
